With your case manager acting as a concierge, it’s possible to achieve your health goals, lower the risk of STD transmission, and improve your quality of life. What is BLISS Health’s case management program?

Case management at BLISS Health provides a concierge for each and every patient. Your concierge acts as a point of contact, helping you access comprehensive care, including doctor’s visits, lab work, mental health services, and prescription medication.

The case management team can also put you in contact with community services that provide transportation, housing, food, and insurance.

Case management begins with a consultation in-person or via telemedicine. If you already have insurance, you can start participating right away. If you’re uninsured or underinsured, we can help you gather the necessary information to help you apply for a subsidy. We assist you throughout the entire process, ensuring you complete each step as required.


What services does case management provide?

Case management at BLISS Health acts as a liaison to facilitate comprehensive medical care to patients, including:

Who qualifies to interact with case management?

If you’re a patient at BLISS Health, you can communicate with our case management team.

Following an evaluation and a review of your financial status, the case management team can make recommendations that align with your needs.

Once you’re enrolled and in communications with the case management team, you can reach out to your case manager at any time. We work with you one-on-one to ensure positive treatment outcomes and a better quality of life.
