What is BLISS Health’s case management program?
Case management at BLISS Health provides a concierge for each and every patient. Your concierge acts as a point of contact, helping you access comprehensive care, including doctor’s visits, lab work, mental health services, and prescription medication.
The case management team can also put you in contact with community services that provide transportation, housing, food, and insurance.
Case management begins with a consultation in-person or via telemedicine. If you already have insurance, you can start participating right away. If you’re uninsured or underinsured, we can help you gather the necessary information to help you apply for a subsidy. We assist you throughout the entire process, ensuring you complete each step as required.
